This has been a very busy year for the North Perth Family Health Team. In November of this year, we implemented a new electronic medical record, Accuro. Planning for this project has taken about one year and included a close examination of our process and how patient visits are documented with a view to improving how we provide services to improve the patient experience. The new electronic medical record will help our physicians and health professionals to better measure and monitor our outcomes to ensure that we focus on continuous quality improvement for you, our patients.

Implementing a new electronic medical record system requires a significant amount of change and so, each of the staff must be part of implementing the new processes and using the new system to streamline and improve how we deliver care and measure our progress. The team at North Perth Family Health Team has demonstrated strong commitment and dedication to our success. We would also like to thank the community of North Perth for their patience during this transition.

Recruitment Program – 2014

The North Perth recruitment program has had a very successful 2014. A strategic goal is to welcome more visiting physicians (locums) and medical learners (Residents and Students) to join us in North Perth to experience firsthand the progressive programs, outstanding facilities and friendly team we offer. A result of this approach was the recruitment of Dr. Lauren Hayward to full-time practice in May 2014. In addition, we had 3 locums who joined us in Listowel for upward of 3 weeks to 3 months. We also welcomed to Listowel four 1st year medical students, five 3rd and 4th year Medical Students and 8 Medical Residents who are physicians specializing in Family Medicine, doubling the number from 2013. Please continue to give a warm welcome to our visiting locums and medical learners as they consider North Perth for long term practice and share our opportunity with colleagues and friends.

Huron Perth Health Link

As the lead agency in the Huron Perth Health Link, our project team has been working tirelessly with our health and social service partners to develop and implement robust communication and structural processes that will allow the care coordination work to continue in a sustainable way in to the future. The project and the working groups have been designed to support the true “heart” of the Health Link – comprehensive coordinated care planning for those identified complex, frail patients. While our initial focus is on this vulnerable population, the structures being put in place will ensure that all patients will receive care that is individualized to their needs. This very important work will transform our health system to deliver high quality and high value health care.

Looking Ahead

We know that 2015 will continue to bring fiscal challenges to all sectors as the government struggles to reduce our ballooning deficit. For North Perth Family Health Team, we remain committed to strengthening our team and improving the experience that you our patients have in delivering the best primary care service.
