Our Health Team

The North Perth FHT uses a collaborative practice model that guides the ongoing development of the FHT as physicians and allied health professionals work together to meet the needs of their patients. A Patient Advisory Committee has be created to provide a forum for the ongoing identification and resolution of issues and problems related to development of the collaborative practice model

1. Family Physicians, Family Health Network

The Listowel Family Health Network is comprised of all physicians in Listowel and has been in operation for several years. Much of the success of the North Perth Family Health Team is due to the close working relationships with the Family Health Network. Their support of the collaborative practice model has contributed significantly in creating a framework to guide the future delivery of primary health care services by the North Perth Family Health Team.

2. Specialists

The family physicians within the NPFHT have access to a full range of medical and surgical specialists. Clients have the ability to access some of these services closer to home.

3. Nurses

Primary Care Nurse Practitioners
Nurse Practitioners are often the first point of contact to clients. They provide a broad range of patient care services within their scope of practice including assessment, diagnosis and treatment, and work closely with all of the family physicians to provide the best possible care to each client. In addition, as part of the Family Health Team (FHT), their expertise contributes to the successful implementation of quality programs and services to the community.

Martha Leonard, RN BScN RN(EC) Primary Care Nurse Practitioner

Martha is an experienced Advanced Nurse Practitioner who joined the North Perth Family Health Team in November 2013. She has worked in health care for over 25 years and enjoys the diversity of care she is able to provide. She is pleased to join this caring team providing primary health services to the community of North Perth and partnering with each of you to achieve your optimal health, from the youngest to the oldest! Martha enjoys being part of the Wayne Gilpin Singers and travelling - including medical mission trips to developing countries.  

Jessie Rumble, RN, MScN  

We are thrilled to welcome Jessie Rumble to our team. She recently graduated from Western University as a Nurse Practitioner and joined us in August 2016. Jessie brings a diverse background including family medicine, health promotion, chronic disease management and addictions.
When not caring for the patients of North Perth, you can find Jessie volunteering with St. John Ambulance, drinking really good coffee, or chasing after her two very busy little boys.

Registered Nurses/Registered Practical Nurses

Registered Nurses and Registered Practical Nurses are key members of the FHT as they develop programs and services tailored to the needs of the community. Either directly or together with the Interdisciplinary Health Care Professionals and other community service providers, they are instrumental in delivering a broad range of integrated programs targeted at the prevention and early identification of disease.

Sarah Givens, RN CON (CNurse Navigator

As a Registered Nurse with the Family Health Team, Sarah helps people with cancer related diagnosis or questions; and their families; navigate through the medical system. Sarah also sits on the Southwest End of Life Network Steering Committee and is a liaison with the Breast Health Program at the Listowel Memorial Hospital. Sarah enjoys being an active member of the North Perth Community and is proud to volunteer her time for The Terry Fox Run, Relay for Life and local school and church activities.

Ruth Anne Biehn, R N., C.B.E., L.C. Lactation Consultant, Prenatal

As a Registered Nurse with the North Perth Family Health Team (NPFHT), Ruth Anne's specialty is in Maternal- Child and Newborn Care. She worked in the obstetric ward at Listowel Memorial Hospital for more than 20 years where she initiated and taught prenatal classes. She has represented North Perth as a member of the Prenatal Health Fair Steering Committeee as well as the Healthy Start for Perth County Working Group . She has recently joined the Reproductive Health and Breastfeeding Network.

Ruth Anne enjoys the rewards of a wonderful husband, three children and seven grandchildren. She is looking forward to the opportunities that working at the NPFHT will provide.
 
Colleen Young, RN  Primary Care Nurse - Triage

Colleen joined the North Perth Family Health Team in November 2008.  She graduated from Conestoga College in 1989 and worked for 20 years at Listowel Memorial Hospital in Obstetrics, ER and the med/surg department.  She has also been a certified CPR instructor for the past 8 years.  Colleen is currently working as a triage nurse at the Listowel Clinic.
         
Patti Rozendal, RN Primary Care Nurse - Triage

Patti joined the North Perth Family Health Team in the Fall of 2016. She was raised and went to school in North Perth and now is married with 3  grown children and boasts a very spoiled grandchild.

She has held various nursing positions over the last 30 years which includes: nursing home, hospital medical-surgical nursing, and most recently in the Emergency Department.

She is very excited to join the NPFHT and to be a part of the primary care environment.  


Patti Graham, RPN
  Primary Care Nurse

Resident of Listowel for over 23 years, Patti has a varied background in health care.  She has over 32 years of experience in nursing as a Registered Practical Nurse, from settings in nursing homes, hospitals, community and with the Listowel Clinic.

Christine Jamieson, RPN  Primary Care Nurse

  Christine Jamieson joined the North Perth Family Health Team as a Registered Practical Nurse in August 2007. A Listowel native, she started her love for nursing while attending Listowel District Secondary School and working as a student at the Brunner Nursing Home. Christine loves the diversity and challenges that the Family Health Team has brought to her career.
 

Llori Nicholls, RPN Primary Care Nurse 

We would like to introduce Llori. Her pleasant demeanour and friendly face is very familiar throughout Perth County as she has worked and resided in the North Perth area for over 15 years with her husband, son, daughter and not to be ignored, very friendly geriatric dog, Luke. Welcome to the team, Llori

 

4. Interdisciplinary Health Care Professionals

The Interdisciplinary Health Care Professionals are members of the NPFHT who participate in direct patient care as well as in core primary care services. These professionals have expertise in a variety of health care disciplines, which fosters quality care as they see individuals and families and as they work together in teams to deliver quality integrated programs and services to the community for the purpose of prevention and early identification of disease.


Joanne Fox, PharmD., B.Sc.Phm   Clinical Pharmacist

Joanne Fox is an Independent Clinical Pharmacist having practiced in a variety of locales in Midwestern, Ontario including both Listowel and Wingham sites of the Listowel and Wingham Hospital Alliance.  Joanne holds a Bachelor of Science in Pharmacy from the University of Toronto, a General Hospital Residency from Kingston General Hospital and a Doctorate in Clinical Pharmacy from Wayne State University, Detroit, MI.  Her greatest strength as a Clinical Pharmacist is her love of teaching and her ability to convey ideas in an easily understood manner.
              
 
Gabrielle Bisson, MSW, RSW,  Social Worker

Gabrielle joined the North Perth Family Health Team in February 2009. She has her Master’s Degree of Social Work (Lakehead University) and has had previous experience working with children and youth, adults and families in a variety of settings. Outside of work, Gabrielle enjoys running and hiking along with embracing the joys of motherhood. Gabrielle is excited to be a part of the North Perth Family Health Team!
 

Laurie Erb, BA, MTS, RP  Registered Psychotherapist
Since the spring of 2008, the North Perth Family Health Team has been able to offer the residents of North Perth with Laurie's expertise in rural mental health. Coming from a rural background, Laurie has first hand understanding of the needs of the rural community. She is a great asset to our team and community.
Heather Mann, BASc,MBA  Registered Dietitian Retired
Heather has been with the North Perth Family Health Team since 2008. She has been involved with individual and group education sessions promoting healthier lifestyles and management of chronic diseases, including diabetes and heart disease. If you have a question about nutrition, Heather is here to help.

Lindsay McGee, RN
  Quality Manager
The North Perth Family Health Team has appreciated the efforts of Lindsay with her background in Quality in the hospital sector. She is very busy collecting stats and creating directives for clinicians and physicians alike to help them better understand how to provide an even higher quality of care for the residents of North Perth.
 
Brittany Dewing,  Choices for Change Youth Worker
At the Family Health Team, we are thrilled to announce the arrival of Brittany to our team and the town of North Perth. She is taking over where Steve Whalen left off - counselling kids and youth about drug and substance addictions, both here at the clinic and at Listowel District High School.  Please make her feel welcome.
 
5. Team Support

The administrative staff of the NPFHT provides support to the Interdisciplinary Health Care Professionals in order to assist in the delivery of programs and services to the residents of North Perth. They help to administer quality services that meet the needs of our community in an efficient and highly effective manner.
 
Mary Atkinson, RN, B.Sc., MBA, CHE  matkinson@npfht.ca  Executive Director

Mary joined the North Perth Family Health Team as the Executive Director in April 2009. She brings a varied background to the position from mental health nursing to managing community home services agencies. Her experience also includes providing marketing and business planning expertise to health services and health informatics organizations. Her academic background includes her RN diploma, bachelors of Science in biology and a master’s in business administration. Mary is committed to providing strong leadership and support to the family health team in the delivery of quality health services to the community of North Perth.

Fran Moore, CGA, Hon. B. Comm. fmoore@npfht.ca Business Manager

Fran joined the North Perth Family Health Team as Business Manager in March 2008. Prior to joining the family health team, she has held positions mainly in the manufacturing sector as a management accountant and is experienced in financial performance measurement, financial reporting and controls.  She holds an Honours Bachelor of Commerce Degree and is a Certified General Accountant (CGA).  Fran’s expertise provides great support to the operations of the organization and plays a key role in decision support.
    
Maribeth Boshart, mboshart@npfht.ca  Executive Assistant
 
Maribeth joined the North Perth Family Health Team in January 2009. Previous to joining the team, she worked as an Optometric Assistant for seven and a half years, assisting the optometrists with promoting and educating patients on their eye health and care. This role, with the Family Health Team seemed to be a good fit with the health promotion and prevention background that she has been involved with. Her creative skills have been utilized in the development of program promotional materials and quarterly newsletters and, certainly, the baked treats are well received at the various meetings where she records the minutes.
