Our Health Team

The North Perth FHT uses a collaborative practice model that guides the ongoing development of the FHT as physicians and allied health professionals work together to meet the needs of their patients. A Patient Advisory Committee has be created to provide a forum for the ongoing identification and resolution of issues and problems related to development of the collaborative practice model

1. Family Physicians, Family Health Network

The Listowel Family Health Network is comprised of all physicians in Listowel and has been in operation for several years. Much of the success of the North Perth Family Health Team is due to the close working relationships with the Family Health Network. Their support of the collaborative practice model has contributed significantly in creating a framework to guide the future delivery of primary health care services by the North Perth Family Health Team.

2. Specialists

The family physicians within the NPFHT have access to a full range of medical and surgical specialists. Clients have the ability to access some of these services closer to home.

3. Nurses

Primary Care Nurse Practitioners
Nurse Practitioners are often the first point of contact to clients. They provide a broad range of patient care services within their scope of practice including assessment, diagnosis and treatment, and work closely with all of the family physicians to provide the best possible care to each client. In addition, as part of the Family Health Team (FHT), their expertise contributes to the successful implementation of quality programs and services to the community.

             Julie Murtha, NP-PHC, MScN 

Julie graduated as a Nurse Practitioner in 2016 and began working with us in August 2018. Her nursing background had a focus on community care and geriatrics, but she enjoys seeing patients of all ages. She lives locally and when not seeing patients at the clinic, is busy with her own two children. She is also an active member of the community, participating in 100 Women who Care North Perth, and local school and church activities. Julie is excited to be a part of her local health care team, and to work in the primary care environment.

Daniel Goodman, NP

Registered Nurses/Registered Practical Nurses

Registered Nurses and Registered Practical Nurses are key members of the FHT as they develop programs and services tailored to the needs of the community. Either directly or together with the Interdisciplinary Health Care Professionals and other community service providers, they are instrumental in delivering a broad range of integrated programs targeted at the prevention and early identification of disease.

Resident of Listowel for over 23 years, Patti has a varied background in health care.  She has over 32 years of experience in nursing as a Registered Practical Nurse, from settings in nursing homes, hospitals, community and with the Listowel Clinic.

Christine Jamieson, RPN  Primary Care Nurse

Christine Jamieson joined the North Perth Family Health Team as a Registered Practical Nurse in August 2007. A Listowel native, she started her love for nursing while attending Listowel District Secondary School and working as a student at the Brunner Nursing Home. Christine loves the diversity and challenges that the Family Health Team has brought to her career.

4. Interdisciplinary Health Care Professionals

The Interdisciplinary Health Care Professionals are members of the NPFHT who participate in direct patient care as well as in core primary care services. These professionals have expertise in a variety of health care disciplines, which fosters quality care as they see individuals and families and as they work together in teams to deliver quality integrated programs and services to the community for the purpose of prevention and early identification of disease.

Joanne Fox, PharmD., B.Sc.Phm   Clinical Pharmacist

Joanne Fox is an Independent Clinical Pharmacist having practiced in a variety of locales in Midwestern, Ontario including both Listowel and Wingham sites of the Listowel and Wingham Hospital Alliance.  Joanne holds a Bachelor of Science in Pharmacy from the University of Toronto, a General Hospital Residency from Kingston General Hospital and a Doctorate in Clinical Pharmacy from Wayne State University, Detroit, MI.  Her greatest strength as a Clinical Pharmacist is her love of teaching and her ability to convey ideas in an easily understood manner.
Sue Qureshi, Child and Youth Counsellor
Sue has been with the family health team, officially since Fall of 2018. She joins us with a wealth of knowledge and experience to empower our young and teen aged patients to work through those difficult issues. Her practice primarily focuses on Child and Youth Mental Health but will see young adults as needed. We are happy to have her aboard our team.
Laurie Erb, BA, MTS, RP  Registered Psychotherapist
Since the spring of 2008, the North Perth Family Health Team has been able to offer the residents of North Perth with Laurie's expertise in rural mental health. Coming from a rural background, Laurie has first hand understanding of the needs of the rural community. She is a great asset to our team and community.
Jenessa Dalton,  Registered Dietitian (Mat leave) Brooklyn Brubacher covering

Jenessa started working with the North Perth FHT in October 2017. She has worked in a variety of practice settings as a Registered Dietitian such as long-term care, home care, and Diabetes Education Centre. She enjoys helping individuals make healthier lifestyle choices. Jenessa sees patients on an individual level and also offers group education sessions.     In her spare time, she enjoys playing a variety of sports, cooking and eating, traveling and reading.

Lindsay McGee, RN  Quality Manager

The North Perth Family Health Team has appreciated the efforts of Lindsay with her background in Quality in the hospital sector. She is very busy collecting stats and creating directives for clinicians and physicians alike to help them better understand how to provide an even higher quality of care for the residents of North Perth.
Brittany Dewing,  Choices for Change Youth Worker

At the Family Health Team, we are thrilled to have Brittany on our team and the town of North Perth. She spends most of her time at the Listowel District Secondary School working with and educating youth about drug and substance addictions. She will see students here at the clinic as well. Our North Perth Youth are lucky to have her. 
5. Team Support

The administrative staff of the NPFHT provides support to the Interdisciplinary Health Care Professionals in order to assist in the delivery of programs and services to the residents of North Perth. They help to administer quality services that meet the needs of our community in an efficient and highly effective manner.
Mary Atkinson, RN, B.Sc., MBA, CHE  Executive Director

Mary joined the North Perth Family Health Team as the Executive Director in April 2009. She brings a varied background to the position from mental health nursing to managing community home services agencies. Her experience also includes providing marketing and business planning expertise to health services and health informatics organizations. Her academic background includes her RN diploma, bachelors of Science in biology and a master’s in business administration. Mary is committed to providing strong leadership and support to the family health team in the delivery of quality health services to the community of North Perth.

Fran Moore, CPA, CGA, Hon. B. Comm. Business Manager

Fran joined the North Perth Family Health Team as Business Manager in March 2008. Prior to joining the family health team, she has held positions mainly in the manufacturing sector as a management accountant and is experienced in financial performance measurement, financial reporting and controls.  She holds an Honours Bachelor of Commerce Degree and is a Certified General Accountant (CGA) as well as a Chartered Professional Accountant (CPA).  Fran’s expertise provides great support to the operations of the organization and plays a key role in decision support.
Jenn Koch, Executive Assistant
Jenn joined the North Perth Family Health Team in April 2019. Previous to joining the team, she worked as a Medical Office Assistant for nine years, at the Listowel Clinic assisting the patients with the referral process.
